Application
Proparacaine hydrochloride and tetracaine are also indicated to produce local anesthesia prior to surgical procedures such as cataract extraction and pterygium excision, usually as an adjunct to locally injected anesthetics.
Ophthalmic solutions used for intraocular procedures should be preservative-free. Preservatives may cause damage to the corneal epithelium if a significant quantity of solutionenters the eye through the incision.
Bupivacaine is indicated for local infiltration, peripheral nerve block, sympathetic nerve block, and epidural and caudal blocks. It is sometimes used in combination with epinephrine to prevent systemic absorption and extend the duration of action. The 0.75% (most concentrated) formulation is used in retrobulbar block. It is the most commonly used local anesthetic in epidural anesthesia during labor, as well as in postoperative pain management&period.
Dosage
(1) brachial plexus block, 0.25% solution, 20-30 ml or 0.375% solution, 20 ml (50-75 mg);
(2) caudal block, 0.25% solution, 15-30 ml (37.5-75.0 mg), or 0.5% solution, 15-20 ml (75-100 mg)
(3) epidural space block, 0.25% -0.375% can be analgesic, 0.5% can be used for general abdominal surgery.
(4) local infiltration, the total amount of 175-200mg (0.25%, 70-80ml) is generally limited, within 24 hours of graded administration, a very large amount of 400mg;
(5) the total amount of sympathetic ganglia block 50-125mg (0.25%, 20-50ml);
(6) subarachnoid block, commonly used amount of 5-15mg, and add 10% glucose into high-density liquid or diluted into approximately equal density fluid with cerebrospinal fluid.
